A 29-year-old man presented with shortness of breath, generalized abdominal pain, and fatigue. Abdominal ultrasound scan showed ascites (A, yellow arrow heads) with loculations (A, white arrows) in the pelvis and the left iliac fossa. The peritoneum (B, white arrow heads) and omentum (B, empty arrow heads) were thickened. Peritoneal and pleural fluid cytology were suggestive of an inflammatory tuberculous process.
