A 29-year-old woman presented with epigastric and lower abdominal pain, fresh vaginal bleeding, and weight loss. Coronal CT scan of the abdomen with intravenous contrast revealed a non-enhanced complex mass lesion measures 4 × 2.5 cm anterior and to the right of the IVC at the level of L3-L4 vertebrae suggesting matted lymph nodes (arrows). Acid Fast Bacilli smear from the mass was positive
